At the Visiting Nurse Service of New York (VNSNY), we are dedicated to providing high-quality health care in the home and the community. We've been a vital part of New Yorkers' lives since 1893, with a tradition of vision and action that has taken our industry in new and important directions. In 2016, we served more than 140,000 patients in the five boroughs of New York City, Nassau, Suffolk and Westchester Counties. Our patients range from newborns to the oldest of the elderly (100+), and reflect the socio-economic and cultural diversity of the New York metro area. VNSNY CHOICE Health Plans, our managed care affiliate, shares VNSNY's mission of compassion and care for the poor, the chronically ill and elderly helping them to live safely and independently for as long as possible in their own homes and communities. On any given day, VNSNY CHOICE coordinates care for more than 40,000 members through its Medicare and Medicaid health plans, including Medicaid Long Term Care, Medicare Advantage, and a HIV/Special Needs Plan. VNSNY provides skilled nursing, rehabilitation therapy, social work and personal care services through a variety of programs including: Home Health Care Post-Acute Care Services Children and Family Services Community Mental Health Services Population Health Home Health Intake Coordination Hospice and Palliative Care Maternity, Newborn, and Pediatrics Nurse-Family Partnership Program Rehabilitation Therapies Private Pay Services (Partners in Care) In 2016, VNSNY provided nearly $47 million in charitable care and community benefit, which included $9.6 million to provide direct home care services to more than 8,600 under and uninsured New Yorkers. These charitable care dollars also went to pay for medications for the neediest; wigs for cancer patients, and home-safety items for the frail elderly.
