Alzheimer's Community Care is a Florida-based 501(c)(3), not-for-profit organization founded in 1996 to provide specialized care to Alzheimer's disease and related disorders patients and to support the needs of caregivers. We serve Palm Beach, Martin and Saint Lucie counties. Alzheimer's disease is an irreversible, progressive brain disorder that occurs gradually and results in memory loss, changes in behavior and personality, a decline in thinking abilities and eventually, death. More than five million people in the United States alone currently suffer with Alzheimer's disease. On average, an Alzheimer's disease patient lives 8 to 10 years after they are diagnosed. Some patients live with the disease for as long as 20 years. The caregivers of patients experience their own stages of the disease (Denial, Anger, Bargaining, Depression and Acceptance). Understanding the dynamics of the disease process and obtaining sound advice is imperative for ensuring a quality of life for both patients and caregivers. Alzheimer's Community Care has developed a proven model of care to combat the difficult effects of Alzheimer's disease and related dementias. Alzheimer's Community Care provides Specialized Alzheimer's Day Care Centers, Family Nurse Consultants, Education and Training, 24-Hour Crisis Line, Caregiver Support Groups, Case Management, and Disaster Preparedness. Our unique organization is dedicated to being the ultimate service and resource provider to people struggling with Alzheimer's disease and related disorders.
